• Parental authority form for student aged between 15 and 18 years (pdf 29kb)
  Students between the ages of 15 and 18 years are required to fill in this form in order to access web sites external to Monash when using their student accounts.

• Parental authority form for student aged under 15 years (pdf 39kb)
  Students under the age of 15 years must fill in a Parent/Guardian authorisation of access for students aged under 15 years form in order to have access to computer services through the University. This form is also available on request from our nearest ITS Service Desk.
